Incident Reporting Form

HIEA member companies are committed to operating their facilities in a safe and responsible manner, and in a way, which does not adversely impact our neighbouring communities.

If you wish to contact a HIEA member company regarding the source of an incident, you may do so by either calling the company directly or by completing HIEA's Incident Reporting Form or contacting HIEA's answering service.
Note: Please save a copy of your completed Incident Reporting Form and attach it to an email and send to info@hiea.org

Upon receiving an enquiry, a representative from the HIEA member company will respond as soon as possible and within 24-48 hours.

• The date, time of day and duration the occurance took place
• A description of the occurance (i.e. noise, dust, odour etc.) and your location when you made the observation
• Your contact information ie. name, address and phone number

HIEA member companies often work together to assist residents and, when appropriate, will contact other HIEA member companies when reviewing an incident.

If the incident cannot be attributed to the company in question then we encourage residents to contact the Ministry of the Environment at the number listed on the magnet.
